Wound Care Coordinator - RN

Job Description

We are looking for a compassionate and driven RN to join Clovis Healthcare and Rehab as a Wound Care Coordinator. In this role, you will lead and integrate a comprehensive Skin Health and Wound management program, championing patient-centered care and outcomes.

Position Highlights

• Integrate and lead a fully developed Skin Health and Wound management program.
• Act as a clinical expert for wound, skin, and incontinent products.
• Oversee durable medical equipment related to wound prevention and management.

Requirements

• Must possess an RN license.
• Completion of required orientation and wound training within specified timelines.
• National accredited wound certification or commitment to obtain it within one year.
• Experience with clinical charting, incident reporting, and interprofessional collaboration.
• Strong knowledge of national wound guidelines, nursing home regulations, and Microsoft Office tools.
