Family Nurse Practitioner or Physician Assistant

Job Description

One Medical, a pioneering primary care solution, is seeking a dedicated Family Nurse Practitioner or Physician Assistant to join their innovative team. As a key member of the healthcare delivery system, you will be responsible for managing a diverse patient panel and providing comprehensive care to individuals of all ages.

Responsibilities

• Manage a patient panel with acute, chronic, and well visit needs.
• Deliver care in-office, testing centers, and via tele-health visits.
• Engage in continuous learning through Clinical Rounds and other modalities.
• Collaborate with in-office and virtual clinical teams.
• Utilize specific clinical training to perform in-office procedures.

Requirements

• Completed accredited FNP or PA program with national certification.
• 2+ years of practice in outpatient primary care.
• State licensed in New York by start date.
• Demonstrated proficiency in evidence-based primary care.
• Effective communication and human-centered care skills.
• Ability to adapt behavior to meet patient needs.
