Job Description
We are currently seeking experienced Home Care LPNs to join our team at ChristianaCare. As an integral part of our HomeHealth division, you will provide skilled nursing care and treatment to geriatric patients in the community. Our organization is well-equipped with a state-of-the-art Electronic Health Record system and offers comprehensive OASIS-E training to support your career growth.
Responsibilities
- Deliver skilled nursing care and treatment to geriatric patients.
- Implement patient-centered care plans in collaboration with the healthcare team.
- Participate in discharge planning for patients.
Requirements
- High School Diploma (HSD) or GED.
- Current DE LPN license or compact state license.
