Job Description
We are looking for an experienced Home Care LPN to join our dedicated team at ChristianaCare. This role involves providing skilled nursing care to geriatric patients in the community, implementing care plans, and participating in discharge planning.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients.
- Implement patient-centered care plans.
- Participate in developing discharge plans.
Requirements
- High School Diploma or GED.
- Current DE LPN license or compact state license.
