As an LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process by completing essential items on the Minimum Data Set (MDS) to ensure proper patient care and accurate reimbursement.

Position Highlights

• Lead the interdisciplinary Resident Assessment Instrument (RAI) process in adherence to guidelines.
• Develop individualized resident Care Plans with a focus on resident needs and desired outcomes.
• Provide training to new team members on clinical content and electronic completion of MDS items.

Requirements

• Active LPN license in good standing.
• Prior experience in a healthcare setting.
• Familiarity with nursing practices, laws, and regulations.
• Proficiency in computer applications, especially Windows systems.
