Minimal Data Set Coordinator - Licensed Practical Nurse

Job Description

Genesis Health is looking for a dedicated Minimal Data Set Coordinator who is a Licensed Practical Nurse to join our team. In this role, you will be responsible for participating in the Resident Assessment Process through the completion of essential items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure appropriate patient care and nursing center reimbursement.

Position Highlights

• Direct the RAI process, ensuring compliance with completion guidelines and electronic encoding and transmission of the MDS.
• Contribute to the development of individualized resident Care Plans in accordance with Federal and State regulations.
• Provide training on clinical content and electronic completion of MDS items to new interdisciplinary team members.

Requirements

• Must be an LPN in good standing and currently licensed by the state.
• Prior experience in a hospital, long-term care facility, or related healthcare settings.
• Familiarity with general, rehabilitative, and restorative nursing practices, medical procedures, and relevant regulations.
• Computer skills, including proficiency with Windows applications.
• AccuMed experience preferred.
• Previous experience as an MDS Coordinator is a plus.
• Proficiency in RAI/PPS processes.
• Current BLS/CPR certification.
