Genesis Health, a leading provider of healthcare services, is seeking a dedicated LPN to join our team as a Minimal Data Set (MDS) Coordinator. In this role, you will play a crucial part in the Resident Assessment Process by completing essential items on the MDS and Care Area Assessments (CAAs) to ensure proper patient care and nursing center reimbursement.

Position Highlights:

• Direct the RAI process and ensure accurate completion and transmission of the MDS.
• Develop individualized resident Care Plans in compliance with regulations.
• Train interdisciplinary team members on clinical content and electronic MDS completion.

Requirements:

• LPN in good standing with state licensure.
• Experience in healthcare facilities.
• Proficiency in general nursing practices and MDS completion.
• Computer skills, AccuMed proficiency preferred.
• Prior MDS Coordinator experience.
• BLS/CPR certification.
