As a Physical Therapist, you will conduct home visits, assess patient conditions, and provide direct care to support rehabilitation.

Responsibilities

• Conduct home visit assessments and provide direct patient care.
• Document patient information accurately utilizing EMR systems.
• Maintain an unencumbered Physical Therapy license for New York.

Requirements

• Graduation from an accredited Physical Therapy program.
• Unencumbered Physical Therapy license for New York.
• Proficiency in EMR documentation.
• CPR Certification.
