Responsibilities

• Conduct in-home Physical Therapy assessments for patients to evaluate their conditions and develop rehabilitation plans.
• Utilize electronic medical records for accurate documentation of patient progress.
• Maintain CPR certification.

Requirements

• Graduated from an accredited Physical Therapy program.
• Licensed Physical Therapist in New York.
• Familiarity with EMR systems, Hello Note experience preferred.
