Responsibilities

• Conduct home visit assessments of patients.
• Provide direct patient care to assess medical conditions and functional capabilities.
• Document patient information accurately in the EMR system.
• Collaborate with a compassionate team to ensure patients' needs are met.

Requirements

• Graduated from an accredited Physical Therapy (PT) program.
• Hold an unencumbered Physical Therapy (PT) license for New York.
• Proficient in EMR documentation.
• Current CPR Certification.
