Paradigm Rehab is seeking a dedicated Physical Therapist to join our compassionate team. As a Physical Therapist, you will be responsible for conducting assessments, providing direct patient care, and facilitating rehabilitation to enhance the quality of life for our patients.

Responsibilities

• Graduate of an accredited Physical Therapy program.
• Conduct home visit assessments to evaluate patients' conditions and rehabilitative potential.
• Proficient in EMR documentation.
• Maintain an unencumbered Physical Therapy license for New York.

Requirements

• Unencumbered Physical Therapy license for New York.
• CPR Certification.
