Responsibilities

• Conduct home visit Physical Therapy assessments to evaluate patients' medical conditions and functional capabilities.
• Provide direct patient care and develop personalized treatment plans to assist in rehabilitation.
• Utilize EMR documentation for accurate patient records.
• Collaborate with patients and families to ensure effective communication and progress tracking.

Requirements

• Graduated from an accredited Physical Therapy (PT) program.
• Possess an unencumbered Physical Therapy (PT) license for New York.
• Proficient in conducting assessments and developing treatment plans.
• CPR Certification.
