Physical Therapist Requirements

• Graduated from an accredited Physical Therapy (PT) college or university.
• Hold a valid, unencumbered Physical Therapy (PT) license for New York.
• Conduct home visit assessments, assess medical conditions, and document patient care.
• Proficient in EMR documentation; experience with Hello Note is a plus.
• Hold CPR Certification.
