Physical Therapy Specialist Responsibilities:

• Conduct comprehensive home visit assessments for referred patients.
• Provide direct patient care, assess medical conditions, functional capabilities, and potential for rehabilitation.
• Document patient information using EMR systems.

Requirements:

• Graduation from an accredited Physical Therapy program.
• Valid Physical Therapy license for New York.
• Proficiency in EMR documentation.
• CPR Certification.
