Responsibilities

• Conduct home visit Physical Therapy assessments to evaluate patients' medical conditions and rehabilitation potential.
• Provide direct patient care and develop treatment plans to enhance functional capabilities.
• Utilize EMR documentation to maintain accurate records of patient progress.

Requirements

• Graduate from an accredited Physical Therapy program.
• Hold a valid Physical Therapy license for New York.
• Proficiency in EMR documentation is a plus.
• CPR Certification.
