Responsibilities

• Conduct thorough home visit assessments of patients to evaluate medical conditions and rehabilitation needs.
• Provide direct patient care to enhance functional capabilities and promote independence.
• Document patient progress accurately using EMR systems.
• Collaborate with interdisciplinary teams to develop personalized treatment plans.

Requirements

• Graduated from an accredited Physical Therapy (PT) program.
• Hold an unencumbered Physical Therapy (PT) license for New York.
• Proficient in conducting assessments, developing treatment plans, and documenting patient progress.
• Current CPR certification.
