Responsibilities

• Conduct Physical Therapy assessments of referred patients in home settings.
• Provide direct patient care to assess medical conditions and functional capabilities.
• Document patient information accurately using EMR systems.
• Collaborate with patients to establish rehabilitation goals.

Requirements

• Graduation from an accredited Physical Therapy program.
• Valid Physical Therapy license for New York.
• Proficiency in EMR documentation.
• CPR Certification.
